feat(story-lifecycle): collision-safe, all-or-nothing ledger writes

Route every .stories/ write through skills/story-lifecycle/scripts/ledger.js:
- exclusive per-ledger lock; a second writer waits, then exits 3 without writing
- story IDs reserved with exclusive-create markers in the git common dir, so
concurrent agents and worktrees of one clone never share an ID
- journaled batches: stage, journal, rename; derived epic/sprint tables are
regenerated inside the batch; an interrupted batch is rolled back by the
next write command or `recover`; `status` reports it only when no live
  writer holds the lock

Bound the ledger to an opt-in planning projection that is never canonical
ECC Task/Evidence/Completion/Integration state, and state that .stories/
follows the checkout.

tests/docs/story-lifecycle-ledger.test.js exercises create, concurrent
create, concurrent cross-worktree allocation, lock contention, overwrite
refusal, interrupted-batch recovery, sprint planning, status drift and
reconcile, and path confinement. Also refresh the skill-triggers manifest
digest for 294 skills.

`.stories/` is an **opt-in planning projection**. It is never canonical ECC Task, Evidence,
Completion, or Integration state. Where ECC execution-capsule contracts, an issue tracker,
or another external task/evidence system is configured, that system stays authoritative:
the ledger only mirrors outcomes that system has already recorded, and it never asserts
completion or integration on its own.
`.stories/` follows the checkout. Another branch shows that branch's ledger, and stories
created on an unmerged branch are not visible elsewhere until that branch is merged.

- **No silent overwrites.** If a target file already exists (epic slug collision, story ID

show the user what would change and get explicit approval before writing.
- **No silent overwrites.** Before updating an existing file (re-planning an existing

Use the active harness's native filesystem capabilities for all `.stories/` access —
not POSIX shell commands. Before the first write, create and verify the exact `.stories/`,
`.stories/epics/`, and `.stories/sprints/` directories with a portable native directory
operation. If the harness cannot create directories, stop and ask the user to create those
exact paths; do not silently fall back to a platform-specific shell command.
## Writes go through the ledger helper
Every write to `.stories/` goes through `scripts/ledger.js` in this skill's directory
(`node <skill-dir>/scripts/ledger.js <command> --root <repo-root>`). Never write ledger
files directly with editor or shell tools: direct writes bypass the lock, ID allocation,
and rollback. The helper is plain Node.js with no dependencies and runs on Windows, macOS,
and Linux. If `node` is not available, use the ledger read-only and tell the user.
| Command | Effect |
| --- | --- |
| `init` | Create `.stories/`, `epics/`, `sprints/` and `.stories/.gitignore` |
| `allocate --epic <slug> --count <n>` | Reserve `n` new story IDs atomically; prints `{ "ids": [...] }` |
| `apply --input <file.json>` | Write one batch `{ "create": { path: content }, "update": { path: content } }` |
| `status` | Read-only: stories, `drift` (derived tables that disagree), `recoveryPending` |
| `reconcile` | Rewrite drifted epic/sprint tables from the story files |
| `recover` | Roll back an interrupted batch |
Paths in `apply` are relative to `.stories/` (`epics/<slug>.md`, `sprints/sprint-<n>.md`,
`<story-id>.md`). Write the input JSON outside `.stories/` (for example in the harness's
scratch or temp directory) and delete it afterwards.
Guarantees the helper enforces:
- **Exclusive lock.** Each write command holds `.stories/.lock` (created with exclusive
create) only for the duration of that command. A second writer waits and retries; on
timeout it exits with code `3` and writes nothing. Rerun it later; never delete a lock
held by a live process.
- **Collision-safe IDs.** `allocate` reserves each ID with an exclusive-create marker in the
git common directory, so concurrent agents and all worktrees of one clone never receive
the same ID. `apply` refuses story files whose ID was not allocated, and refuses to
create a file that already exists (exit code `4`).
- **All-or-nothing batches.** `apply` stages every file, writes a journal, then moves the
files into place. The derived epic and sprint tables are regenerated inside the same
batch. If a process dies mid-batch, the next write command (or `recover`) restores every
backed-up file and deletes every file the batch created. Until then, `status` reports
`recoveryPending: true`, and the next write command restores a consistent ledger
before it does anything else.

## State model
The **story file is the single authoritative record** of a story's status. The stories
tables in epic and sprint files are derived summaries — convenient to read, never
authoritative. Whenever a user-approved story transition changes status, regenerate the
matching rows in the epic file and (if the story is assigned) the sprint file from the
story files as part of that same approved operation. Plain `status` is read-only: on any
disagreement, the story file wins, but show the proposed table diff and require explicit
approval (or `status --fix` plus confirmation) before writing the reconciliation.
